The application process for both programs is now online. There are several parts to this process, and Application Checklist will guide you through the steps. The application deadline for Fall 2022 admission is December 3, 2021. Incomplete applications will not be considered. We recommend that you download the forms and collect all required documents before you begin the online application form.

Download Disability Documentation list and give to special education case manager to obtain the required documentation.

Student Questionnaire
These questions should be answered by the student. A parent or support person may scribe (type), but all answers should be from the student’s perspective.  

Parent Questionnaire
One or both parents may answer these questions. Please be honest and realistic in your answers.

Teacher/Service Provider
Send this link to a teacher or service provider who is currently working with the student and knows them well. This person must be able to speak to the student’s strengths, challenges, and social, emotional, and behavioral functioning. Examples: special education teacher, mental health or behavioral practitioner, disability service or support provider.

We look forward to meeting you when you come to for a visit! Because Aggies Elevated happens all over campus, scheduling a tour is a 2-step process. Please complete both steps to ensure that program staff will be available to meet with you.

Step 1. Schedule a campus visit by clicking on the link. Make sure you indicate either the Logan or Price location. At the bottom of the form, indicate “non-degree seeking” and put “Aggies Elevated” in the comment box. This will reserve your space in the group tour of campus.

Step 2. When scheduling, please be aware that most AE staff members are in class and unavailable before noon. Send an e-mail to (Logan campus) or (Price campus) with the date and time of your campus tour, and request an advising appointment with one of our program staff.

**NOTE: To avoid unnecessary confusion and/or delays on your tour day, do not schedule an exploratory advising appointment in USU's General Advising office. Your appointment will be with an Aggies Elevated staff member.
